The header shows the year as a button — tap it, a scrollable year list drops down, pick the year, then tap the month. One pane, everything one tap deep.
The picker opens on a YEAR grid — pick the year, it flips to that year's months, pick the month, done. Exactly your narration: "you select the year and the month for from, then the year and the month for to." A breadcrumb shows where you are, with a back link.
